2. Your desired name for the t-shirt
3. Your forst 6 digits of your MyKad or IC
4. We need by next weee Monday or Tuesday

The IC number is for the barcode thingie on the shirt, if I'm right. Don't worry about the design; Isaac, Ivan and Fahri made it real cool.
